Before you send it, try these

Quick checks that might save you a repair.

Try a different display cable, and a different port on both the card and the monitor.
Try a different power cable, and connect the card straight to the power supply rather than through a riser.
If you can, test the card in another PC, which confirms the fault is the card and not the rest of the system.
Disable the graphics card's audio device in Windows Device Manager (under Sound, video and game controllers). A faulty audio controller can make the whole card look faulty.
Reinstall your graphics drivers cleanly: use DDU (Display Driver Uninstaller) in Safe Mode to fully remove the current driver, then install the latest stable version fresh.
